TexShare Card Program Statistics Form for the Period September 1, 2013 - August 31, 2014
Use this form to submit statistics for the Card Program.
The TexShare Card Agreement calls for all statistics to be reported annually to the Texas State Library. The statistics should cover September 1, 2013 - August 31, 2014. If you have only participated for a few months, please report whatever use you have had. We ask that you submit a report even if your library has had no TexShare Card activity during FY2014. If that is the case, please enter the number 0 in each field and submit.
Please take a moment to review your library's TexShare Card Lending Policy, which you can locate via the Find_A_Library page. If you wish to make changes or updates, you can use the online Lending Policy Form.

If you wish to leave a statistics field blank, then a number 0 (zero) will be recorded as the defaulted value.
Please make only one entry for your library. It should be an aggregate total for all your library's branches.

There is an ACCD Alamo Community College District form in addition to one for each of 4 campuses (no NLC) There is only one lending policy online ----just the one for the group entry. Contact information varies too. For questions, contact Tracey Mendoza at NLC.
The designated reporter (Mary MacWithey in 2014 and 2015) for the Alamo Colleges collects the data from each school and enters this combined data into the Alamo Colleges form. Data is collected on an Excel spreadsheet sent out to all at the end of August each year. Each school may also enter their data on their own form via the TSLAC email message they received.
In 2015 the data was reported via email to the new contact at the Texas State Library.
